Membership Policies Enrollment & Payments
- Memberships are billed automatically at the beginning of each month.
- Payment methods accepted: credit/debit cards, ACH.
- First month’s payment is due at sign-up.
- Unused visits will not carry over into the next year.
Cancellation Policy
- Notify Optimum Care Clinic at least 30 days prior to your desired cancellation date.
- A $197 reinstatement fee applies if you choose to rejoin after cancellation.
Missed Payments
- If payment is declined, services will be placed on hold until payment is resolved.
- A late fee 0f $25 will apply on the 8th
- Accounts past due by 15 days may be canceled.
